enter a jpsr!

JPSR stands for Joint Patient Safety Reporting. This is the VA's Patient Safety forum where you can enter any error, near miss, close call, safety event, ANYTHING that you feel is getting in the way of a patient's care. This is the way that change is made over time in the hospital.

If you ordered stat antibiotics and it took 6 hours for the patient despite your best efforts - enter a JPSR.

If you ordered an MRI Brain and it took 3 days for it to be done - enter a JPSR.

If you ordered a fluid bolus and it never got done - enter a JPSR.

Obviously, many things in the hospital need more than just an order but a phone call to the nurse or pharmacist or radiology technician to get things done. But if you are doing your due diligence and best work and still running into difficulties, please enter a JPSR.

This is how we are able to track and document trends in the hospital. If Patient Safety sees a trend of events they have the ability to make significant changes. They just can't know the trend exists if you don't enter the events when they happen.

How to enter a JPSR

  1. Go to the DC VA Homepage: https://vaww.washingtondc.va.gov/

  2. Scroll to the bottom and click "Joint Patient Safety Reporting (JPSR)"

  3. Login with your PIV

  4. Click on "Department of Veterans Affairs"

  5. Fill out the form - as much or as little as you'd like. Note you do NOT need to enter your name - this can remain anonymous if you'd like.

  6. Click "Submit."

Please let the chiefs know as well if you are running into any problems in general because we can often help. We can also help enter a JPSR with you if you have any questions.
